Job Description

Salary

The salary range for this position is CAD $42.27/Hr. - CAD $52.81/Hr.

Summary

The Social Worker supports the interdisciplinary team in achieving program patient and resident goals through therapeutic services assessments and coordination of care. Reporting to the designated Social Work Site Leader and receiving guidance from the Professional Practice Leader and/or Research Education and Practice Coordinator the role works under the direction of the Patient Care Manager.

Key Responsibilities:

  • Provides therapeutic clinical counselling services

  • Conducts comprehensive biopsychosocial assessments

  • Maintains patient and resident records

  • Provides consultative and direct services to patients/residents families staff and community health professionals regarding community resources

  • Provides leadership to the interdisciplinary team in the discharge planning process

  • Coordinates clinical and case conferences

  • Develops and delivers in-services and presentations for staff

  • Participates as a member of local committees and group meetings

  • Maintains broad knowledge of developments in the Social Work profession

What You Bring

Education:

  • Masters in Social Work from an accredited university. One year of recent related experience in a designated program area or an equivalent combination of education training and experience.
  • Current full registration with the British Columbia College of Social Workers.
  • Bachelor of Social Work degree from an accredited School of Social Work with an equivalent combination of education training and experience may be considered.

Skills and Abilities

  • Demonstrated ability to communicate with and deal effectively with patients/residents families staff physicians and other health care providers
  • Knowledge of legislation and government policies and procedures related to healthcare

  • Demonstrated ability to gather relevant data and make a biopsychosocial assessment

  • Demonstrated ability to conduct patient/resident assessments

  • Knowledge of other health care disciplines and their role in patient/resident care

  • Demonstrated ability to draw out information that will contribute to understanding the patient/residents situation in order to explore issues beyond the initial concerns

  • Demonstrated ability to work collaboratively as a member of an interdisciplinary team

  • Demonstrated ability to deal with and/or guide others in conflict resolution issues

  • Physical ability to carry out the duties of the position

  • Ability to work independently and prioritize issues and workload

  • Ability to operate related equipment

  • Ability to participate in research

  • Demonstrated ability to provide supervision and teaching to students

  • Ability to facilitate and influence group and inter-group meetings processes and decision making

Your Day to Day

  • Provides therapeutic clinical counselling to patients/residents families and groups; coordinates services and referrals

  • Conducts biopsychosocial assessments by gathering and interpreting patient/family data

  • Maintains documentation including assessments progress notes and required records

  • Participates in discharge planning and liaises with community partners for complex cases

  • Advises patients/families/staff on community resources and access

  • Contributes to care team conferences admissions and discharges

  • Supports understanding of legislative processes (e.g. HCCCFAA MAiD Mental Health Act)

  • Participates in program evaluation and provides social work input on care issues

  • Develops and delivers staff training and in-services

  • Participates in committees and identifies service delivery improvements

  • Facilitates/participates in therapeutic and educational group sessions

  • Stays current in social work practice through literature review and professional development

  • Maintains workload and statistical records; reports trends and variances

  • Conducts approved research and projects; aligns practice with findings

  • Provides supervision and teaching to social work students

  • Performs other related duties as assigned
