Job Description

Are you driven by the opportunity to help others regain mobility confidence and independence in their own homes Were seeking a compassionate and capable Physical Therapist Assistant (PTA) to join a growing home health team serving the West LA area. If youre looking for a flexible purpose-driven role that lets you build meaningful patient relationships outside of a clinic setting this could be a great fit.


What Youll Be Doing


  • Partner with licensed Physical Therapists to implement personalized rehab programs tailored to each patient s goals and needs.

  • Assist patients with mobility-focused activities therapeutic exercises and daily functional tasks.

  • Provide regular follow-up visits to monitor progress adjust treatment strategies and ensure continued eligibility for services.

  • Communicate and coordinate with physicians and interdisciplinary teams to deliver high-quality holistic care.

  • Keep clear accurate documentation in accordance with home health regulations and professional standards.

  • Travel between patient homes offering care where it matters most.



Requirements

What Youll Need to Succeed


  • A valid Physical Therapist Assistant license in the state of California.

  • CPR certification that s up to date.

  • A current California drivers license a clean driving record and dependable transportation for home visits.

  • Availability for a minimum of 25 patient visits per week to be considered full-time.

  • At least two weekdays per week of availability and for PTAs two weekend days per month.



Bonus Points If You Have:

  • Prior experience in a home health setting (not required but preferred).

  • A strong commitment to patient-centered care and collaborative teamwork.
