Direct Support Professional - Community Facilitator

Work Hours: Part-time(hours vary - make your own scheduled based on the clients!)
Primary Location: San Diego County
Compensation: $19.00 per hour

A day in the life of a Facilitator: As a facilitator you will start your day by meeting with the client at an agreed upon facilitator will provide necessary one-on-one support to the client for that days activity and/or event. If the clients goal is to register for school the facilitator will assist in all aspects of signing the client up. If the clients goal is to improve their communication and socialization skills the facilitator will provide support by having the client participate in the community. If the clients objective is employment the facilitator will assist the client in building a resume applying for a job practicing mock interviews and when the client is hired. The facilitator supports the client for at least 3 to 4 hours per session two to three times a week. Lastly before the days shift is over the facilitator and the client will complete a progress note together and sign it.
Job Specific Essential Duties
- Train support and monitor clients while in the community and vocational settings.
- Participate in the development of a circle of support to build relationships with family friends neighbors and other community.
- Develop individualized adaptive support materials as needed.
- Develop and expand clients participation in integrated employment activities.
- Assist with development and implementation of clients annual plan goals and objectives
- Participate in clients annual Individualized Program Plan (IPP) meeting
- Provide same day shift coverage as requested by supervisor.

Minimum Experience andQualifications
- Education: High School Diploma/GED
- Experience:1 year working in social services education or with the developmentally disabled population preferred
- Driving:3 years driving experience
