Job description:

Setting: Skilled Nursing Facility Sub-acute and long-term care patients
Schedule: Minimum of (2) days on site per week. Telehealth support is available on-site for virtual interactions
Must be licensed in VT
Experienced in SNF preferred not required
Potential: 110-225K (Base Salary RVU Bonus Model)
