4Summary:
Utilizing a collaborative process the Case Manager will assess coordinate and implement a medical plan of care for children residing in selected Tarrant County homeless shelters. This individual will be charged with the development and coordination of a program which oversees the health care for designated homeless children treated in the Cook Childrens Health Care System. This position will be responsible for the compilation of data to measure the impact of this program on homeless children and the possible justification for replication in other communities. This individual will perform an ongoing review of the health care management of inpatient homeless children and follow up to monitor compliance with the post discharge plan of care.

Education and Experience
Associate degree in nursing and at least three years of any combination of experiences working in/with case management care coordination utilization review patient intake discharge planning and troubleshooting fund resources quality assurance clinical pathways continuous quality improvement or state and federal health plans or commercial insurance plans in a clinical or managed care environment.
