Job Summary:

Work Schedule: Evening Shift Weekend and Holiday rotation 16 hours weekly

Provide medication therapy management services in collaboration with other providers to assure that drug therapy is safe appropriate cost-effective and efficient. The pharmacist must maintain competency in unique aspects of drug therapy for the patients whom they provide care for including factors related to patients of all age groups (neonates pediatrics adults and geriatrics) and patient disposition (inpatient ambulatory and outpatient).

Pharmacy services provided may include interdisciplinary patient care rounds medication reconciliation medication therapy monitoring medication orders management medication dispensing patient education drug information drug dosing and various scholarly activities. The pharmacist will provide both clinical and distributive tasks for safe medication use and provide training and education to students and residents as necessary.

Mastery of the Cerner pharmacy and hospital information system application is required. Must be proficient with technology and workflows associated with the medication management process including (but not limited to) automated dispensing cabinets anesthesia carts USP <797> IV requirements and clinical decision support software.

The pharmacist ensures that patient care is maintained through compliance with the departments and organizations policies rules and regulations by the respective State Board of Pharmacy and other applicable regulatory bodies.

Responsibilities:

Essential Job Functions

1. Patient Care Services

  • Review medication orders for appropriate dose duration frequency dosage form indication allergy interactions and contraindications.
  • Ensure that medication regimen is consistent with all organizational medication use guidelines and policies or other evidence-based guidelines in the absence of organizational guidelines which includes (but is not limited to): antimicrobial use intravenous to oral transition renal dosing anticoagulation and pharmacokinetics/serum drug concentration protocols.
  • Adjusts medication regimen based on approved protocols and/or consultation with interdisciplinary medication therapy for potential or actual drug interactions (e.g. drug-drug drug-disease drug-allergy food-drug or contraindications) and outcomes.
  • Works as an active member of multidisciplinary team and collaborates with healthcare providers within decentralized patient care areas to provide patient-centered care
  • Participates in interdisciplinary patient care roundsand oversees pharmacy resident and student participation in patient care rounds when applicable.
  • Performs and/or reviews medication histories comparing prescribers orders to the patients active medication list(s) and works with prescribers to make changes as necessary
  • Document activities related to medication reconciliation in medical record across patients transition of care (including but not limited to: admission transfer and discharge)
  • Complete discharge medication review facilitate the discharge process including discharge prescriptions and/or provide discharge education per department procedures or upon request by patient or health care provider.
  • Ensure compliance with health system inpatient drug formulary
  • Completes medication profile review when staffing clinical area reviewing the following factors as appropriate:
    • Age sex health weight renal & hepatic function or other physiological parameters
    • Pregnancy and lactation status
    • Appropriateness of the dose route rate timing & frequency of administration and indication
    • Review for therapeutic duplication
    • Available laboratory values and microbiology results
    • Appropriateness of antimicrobial therapy (including anticipated/planned stop dates)
    • Potential drug interactions and contraindications

2. Coordination of Distribution Systems

  • Coordinate and participate in the preparation and distribution of medications including communication of pending issues/transactions with team members and other health care professionals
  • Coordinate drug delivery and documentation as appropriate.
  • Complete and pass competencies associated with sterile non-sterile and hazardous drug compounding management as assigned based on area of work
  • Understand safety precautions including hazardous materials handling and proper precautions against blood borne pathogens as it relates to area of practice
  • Monitor that the medication use system is providing the right patient the right drug the right dose the right route and the right time with the right outcomes.
  • Serve as a liaison for inpatient patient information for outlying patient needs (retail pharmacies home health physician clinics etc.)

3. Quality Improvement

  • Identify and report adverse drug events (adverse drug reactions and medication errors) in the organizational reporting system
  • Assure compliance with health system hospital and departmental policies and procedures
  • Monitor compliance with state and federal laws and regulations related to the practice of pharmacy as well as standards and regulations for other regulatory agencies
  • Participate in departmental and hospital committees as assigned

4. Training Education and Community Service

  • Provide direct supervision of certified pharmacy technicians pharmacist interns and pharmacy students and assist with their education training and development
  • Provide drug information/education to health care providers and patients upon request and as per department procedures
  • Provide in-services and lectures to health care team members as assigned
  • Precept and evaluate pharmacy residents and PharmD students on residency/clerkship rotations
  • Provide drug information and disease prevention programs to consumer and patient groups as needed
  • Follows and understands the mission vision core values and Employee Standards of Behavior
  • Complete required departmental education/competency modules (Age Friendly etc.).

5. Other duties as assigned

Job Requirements

Education

  • Bachelors degree in Pharmacy or doctor of pharmacy (PharmD) degree required.


Experience

  • Must be at least 18 years of age with no current drug or alcohol addition/convictions of any drug related misdemeanor or felony
  • Two years of health care pharmacy experience preferred.


License/Certifications

  • Current active Pharmacy Licensure (PH) for work state required.
  • American Heart Association Basic life support (BLS) certification preferred.
  • American Heart Association Advanced cardiovascular life support (ACLS) certification preferred.
  • Completion of an ASHP accredited residency training program and/or certification through a certifying program (Board of Pharmacy Specialties etc.) preferred.
