About the role
We are seeking a clinically experienced and motivated professional to provide leadership and oversight of complex health supports across our Supported Independent Living (SIL) services.
Based in Darwin the Clinical Care Coordinator plays a key role in ensuring participants with complex health needs receive safe person-centred and high-quality support. The role includes regular travel to Somervilles SIL services in Palmerston Katherine and Alice Springs supporting teams and strengthening clinical practices across regions.
This hands-on Monday to Friday role offers professional variety autonomy and the opportunity to use your clinical expertise to build team capability and improve outcomes for people with disability.
What youll be doing
As Clinical Care Coordinator youll lead clinical risk management care planning and practice development across multiple SIL houses. Youll also coach frontline staff promote safe and person-centred practices and collaborate with health professionals and families to ensure integrated responsive care.
