3 months contract with local authority
We are seeking a Mental Health Social Worker to fulfill responsibilities including completing Care Act Assessments Reviews Safe guarding and care coordination under the CPA Framework. The successful candidate will manage a caseload of 25 clients and be an integral part of a Duty Rota which may involve screening triaging and handling new referrals. This role will also involve active participation in an integrated Mental Health and Wellness Team.
Requirements
- Bachelors or Masters degree in Social Work or related field
- Relevant professional certification or licensure
- Proven experience in completing Care Act Assessments and Reviews
- Strong understanding of Safe guarding procedures and care coordination under the CPA Framework
- Ability to manage a caseload of 25 clients effectively
- Excellent communication and interpersonal skills
- Ability to work collaboratively in a multidisciplinary team
- Knowledge of mental health and wellness practices and resources
