Job Description
Employment Type:Part Time
Schedule:Flexible Schedule
Reports To:Clinical Director / Nursing Supervisor
- Manage a caseload of hospice patients providing nursing assessments care plans and skilled interventions
- Educate patients and families on disease processes symptom management and end-of-life care
- Coordinate with physicians social workers chaplains and other hospice team members
- Conduct regular visits and respond promptly to changes in patient conditions
- Maintain accurate and timely clinical documentation in accordance with agency policies and regulatory standards
- Support patients and families with emotional spiritual and grief-related needs
- Participate in on-call rotation as scheduled
- Registered Nurse (RN) license in the state of TN
- Minimum of 1 year clinical nursing experience hospice or palliative care preferred
- Strong assessment critical thinking and communication skills
- Compassionate patient-focused approach to care
- Valid drivers license and reliable transportation
- Ability to work independently and as part of an interdisciplinary team
