Job Summary
We are seeking an experienced empathetic and dedicated Nurse Case Manager to join our team. This role is responsible for coordinating and providing skilled nursing care collaborating with the interdisciplinary team and ensuring our patients and their families receive the highest level of support during their time with us.
Responsibilities
- Manage a caseload of patients providing nursing assessments care plans and skilled interventions
- Educate patients and families on disease processes and symptom management
- Coordinate with physicians social workers chaplains and other team members
- Conduct regular visits and respond promptly to changes in patient conditions
- Maintain accurate and timely clinical documentation in accordance with agency policies and regulatory standards
- Support patients and families with their healthcare and emotional needs
- Participate in on-call rotation as scheduled
Qualifications
- Registered Nurse (RN) license in the state of TN
- Minimum of 1 year clinical nursing experience case management hospice or palliative care preferred
- Strong assessment critical thinking and communication skills
- Compassionate patient-focused approach to care
- Valid drivers license and reliable transportation
- Ability to work independently and as part of an interdisciplinary team
