Key Responsibilities: LPN/LVN Direct Patient Care & Nursing Support
- Carries out the medical plan of care and oversees the Nursing Assistants care of residents
- Administers and records medication treatments and monitoring as ordered
- Accurately transcribes physicians orders
- Completes monthly summaries and updates care plans
- Notifies the RN Supervisor of significant changes in residents condition
- Assists physicians with rounds examinations and treatments
- Supervises and directs resident care by nursing assistants
- Documents weights and vital signs and follows up as needed
- Maintains a clean safe and healthy environment
Qualifications: Licensed Practical/Vocational Nurse Certification & Essential Skills
- Current unrestricted Licensed Practical Nurse (LPN) or Licensed Vocational Nurse (LVN) license in the state of practice.
- 1 years of experience in ANY of the following:
- Skilled Nursing Facility Care
- Senior Care
- Home Health
- ANY of the following valid licenses/certifications:
- American Heart Association Basic Life Support (AHA BLS)
- American Red Cross Basic Life Support (ARC BLS)
- American Heart Association Advanced Cardiac Life Support (AHA ACLS)
- ANY of the following valid licenses/certifications:
- Licensed Practical Nurse (LPN) in Pennsylvania (PA)
- Licensed Vocational Nurse (LVN) in Pennsylvania (PA)
Schedule and Locations:
- Nights 6PM - 6AM // Two days on // Two days off // Three days on // 84-hours biweekly pay period. Rotating Weekends.
