Home Health Licensed Practical Nurse (LPN)
Interim HealthCare is currently seeking a compassionate and skilled Licensed Practical Nurse Home Health (LPN) to join our team and provide specialized care for an individual in their home located in Rural Retreat VA area.
Hours: Monday - Friday 8:00 am - 4:00 pm or 12:00 am - 8:00 am We offer a flexible schedule.

As a Home Health Licensed Practical Nurse heres a big-picture view of what youll do:
- Provide home-based care to patients with illnesses injuries and chronic diseasesoften after hospital dischargeunder the supervision of an RN
- Work with a team of physicians RNs CNAs and aides to deliver quality patient care
- Take patient vitals do assessments document progress and report changes to RN supervisor
- Assist RN in carrying out patients plan of care and teaching patient self-care techniques
- Educate family on the patients plan of care medication and home safety
- Collaborate with an interdisciplinary team to coordinate patient care and plan for discharge
A few must-haves for Home Health Licensed Practical Nurses:
- Graduate of an accredited practical nursing program and active LPN license in Virginia
- Minimum of one (1) year of professional experience practicing as LPN experience ideally in home healthcare
- Current CPR/AED/BLS/First Aid certification
- Current Tuberculosistest
- Ability to pass criminal background check
- Knowledge of state and federal home health regulations
- Clinical proficiency in nursing strong communication skills and compassion
