Your role and responsibilities
As a Reablement Support Worker Team Leader youll be at the heart of our mission to help people regain independence and confidence in their own homes.
Youll lead a team of Reablement Support Workers ensuring high-quality person-centred care is delivered in line with each customers Short Term Reablement Plan (STEP). Youll work closely with colleagues across health and social care to make sure services are responsive effective and empowering.

The team
Home First provides short-term reablement care and support to people in their own homesoften following a hospital stay illness or crisis. Our goal is to help people regain independence and avoid unnecessary hospital admissions.
Youll be part of a passionate collaborative team that works closely with Adult Social Care Cluster Teams Independent Providers Health colleagues Borough and District Councils the Multi-Agency Safeguarding Hub and voluntary sector partners.
