Job Description
Clinical Case Manager I
Masters degree in an appropriate area; or a bachelors degree in an appropriate area and two years of relevant experience.
- Follows procedures as established in FL HIV/AIDS Eligibility Procedures Manualfor determination of client eligibility or ineligibility.
- Assists client with understanding the requirements of the eligibility process.
- Educates links and refers clients to follow-up medical care and communityresources to address needs in collaboration with other Ryan White NetworkProviders including referring clients to case management for assistance with anyneeds outside the scope of the eligibility determination process.
- Is familiar with appropriate sources of referral for mental health substanceabuse home care food transportation and other support services for peopleliving with HIV/AIDS. Provides referral assistance with above in addition toinsurance issues Medicaid Medicare and other public assistance services.
- If needed provide linkages to agency community and government services andmonitor referral completion including state insurance continuation programsADAP medication patients assistance programs (PAP) benefits/health coverageprograms such as SSI SSDI Medicaid Medicare housing programs etc.
- Provides documentation of eligibility requirements as define by the HRSA RyanWhite program. Enters accurate documentation of activity through case notesand completion of eligibility forms Maintains and updates client records and notesutilizing a database system designed for tracking of progress notes and otherrequired information reporting as defined by agency standards including electronic medical record and/or CAREWare.
- Attends weekly clinical staffing providing input regarding patient medical case management needs. Attends other divisional meetings as required including weekly research meeting monthly Case Management Co-Op meetings monthly division meetings and other meetings as required.
- Attends education activities trainings and events as required including monthly HIV Grand Rounds Annual HIV Conferencing Medical Case Management training and other trainings as required. Actively participates in community awareness and testing events including National HIV/AIDS Awareness World Aids Week and other awareness events.
$43888 to $51000 annually depending on education and experience.
Masters degree in an appropriate area; or a bachelors degree in an appropriate area and two years of relevant experience.
- Degree in a relevant discipline such as social work nursing public health or public administration.
- HIV/AIDS hepatitis and substance abuse prevention or service coordination experience with the target population preferred.
This is a time-limited position. Time limited positions are based on circumstances such as funding sources which control the length of time for which the position is available.
