Hourly Pay: $21$25/HR DOE
Schedule: Full-Time

What Youll Be Doing:
Assist in meeting residents psychosocial needs
Support care planning documentation and assessments
Coordinate services and communicate with residents families and community partners
Help ensure smooth admissions discharges and transitions
Contribute to a warm supportive and resident-centered environment
Ideal Candidate Will Have:
A passion for helping others and a heart for seniors
Excellent communication and organization skills
Prior experience in a healthcare or social services setting is a plus
