Job Description

Program Description:
Circles of Care is hiring for a knowledgeable and collaborative Substance Use Case Specialistfor ourMedication Assisted Opioid Treatment (MAT) Team. The individual selected will perform intermediate professional work assessing individual needs developing implementing and monitoring treatment service delivery and/or assistance plans; coordinating and monitoring services within the MAT clinic and Circles of Care providers as well as other agencies; Assisting individuals; maintaining records and files; preparing reports and related work as apparent or assigned.
Work is performed under the limited supervision of the Substance Use Services Director and MAT medical professionals.
Completes required new client/patient intake forms prepares BAM and Biopsychosocial reports ASAM/FASAM enrollment.
Develops a clients treatment plan for care services and recovery goals consistent with the treatment plan and clients need; reviews and updates treatment plans as needed and/or required by the managing entity.

Makes referrals and linkages to appropriate agencies for wrap-around services; coordinates client care with outside services in a broad spectrum of community providers and agencies.
Evaluate and document changes in clients condition and counsel individuals as necessary; evaluate individuals environment for safety security negative factors and other barriers to care including individuals knowledge of and ability to solve practical life problems and make successful life choices. Updates treatment plans.
QUALIFICATIONS:
  • Requires a Bachelors Degree in the related field; CAP preferred.
  • Ability to solve problems within the scope of responsibility; analyze facts and exercise sound judgment in arriving at conclusions; ability to communicate complex ideas effectively orally and in writing.
  • Ability to prepare clear and concise reports; ability to establish and maintain effective working relationships with individuals medical professionals community partners agencies associates and the general public.
