HEALTH PLAN COORDINATOR

Job Summary:

In collaboration with the multidisciplinary treatment team oversees the development review/revision of assigned treatment plans ensuring compliance with all licensing and regulatory bodies. Provides case management duties for the patients of the Arizona StateHospital.

Job Duties:

- Coordination of development review and revision of treatment plans in collaboration with the multidisciplinary treatment team ensuring compliance with all licensing and regulatory body requirements for treatment plans.


- Provides notifications of outside agencies when requested by statue rule licensure or certification or policy and completes associated documentation
.


- Assists in case management for coordination of patient admissions and discharges to and from the hospital. This includes but is not limited to the following: contacting outpatient providers completing related hospital forms and procuring releases of information.


- Provides department support according to level of education and credentials (i.e.: psycho-educational familygroup case management data collection administrative tasks education program).


- Other duties as assigned as related to the position (typically 5% - 10%)

Knowledge Skills & Abilities (KSAs):

Knowledge of:

- Understanding of mental illness and basic understanding of current theories of mental
illness and treatment of persons with mental illness.
- Performance Management (PM) and Continuous Quality Improvement (CQI) and
Lean methodologies.

Skill in:

- Excellent verbal and written communication.
- Diplomatic and professional demeanor.
- Strong skill with computer programs in Microsoft office suite and electronic medical
records.

Ability to:

- Work with a diverse patient population.
- Support a diverse multi-cultural workforce that reflects the community promotes equal

environment that enables all individuals to perform to their fullest potential free from
discrimination.

Selective Preference(s):

Candidate will have a Bachelors Degree in Psychology Social Work or related field.
Preference given to applicants with Public Health experience.

Pre-Employment Requirements:

A State Hospital employee must possess a valid level one fingerprint clearance card issues pursuant to A.R.S.41-1758.07 or must apply for a level one fingerprint clearance card within seven working days after beginning employment.

All newly hired State employees are subject to and must successfully complete the Electronic Employment Eligibility Verification Program (E-Verify).
