Primary Care Provider Transitional Care Management (TCM) Focus

Location: Boise ID
Employment Type: Full-Time / Part-Time options available
Compensation: Competitive Fee-for-Service model with ramp-up support

Position Overview

Were seeking an experienced and patient-centered Primary Care Provider Nurse Practitioner Physician Assistant or Physician to lead our Transitional Care Management (TCM) services. Youll be the trusted clinical partner for patients recently discharged from SNFs and continue to support them as they settle back home or into assisted living building lasting relationships that extend into ongoing primary care.

What Youll Do

  • Lead TCM visits and serve as the primary point of contact for patients transitioning out of SNFs leveraging established relationships (e.g. Ensign facilities).
  • Build continuity of care by developing and maintaining patient panels in assisted living communities through the TCM process evolving into regular primary care visits.
  • Coordinate care with facility staff families and other providers to ensure timely follow-up medication reconciliation and preventive services.
  • Promote wellness and address both acute and chronic conditions providing a strong safety net post-discharge.
  • Collaborate with an integrated care team with full administrative and clinical support.

Who You Are

  • Licensed NP PA or MD/DO in Idaho
  • Experienced in transitional care geriatrics primary care or post-acute settings
  • Skilled at relationship-building with patients families and facility partners
  • Organized and comfortable practicing autonomously with flexible scheduling
  • Compassionate communicative and dedicated to patient-centered outcomes
