POSITION SUMMARY:
Provides comprehensive assessment of patients with Diabetes including planning implementation coordinating monitoring and evaluation of a plan of care. Serves as a resource for Diabetes care across the continuum of care. Functions as a practitioner consultant role model and educator ensuring quality patient care and outcomes. Coordinates optimal resources within the health care team of physicians nurse managers clinical specialists patients and families. Participates in continuous quality improvement activities and educational experience in support of departmental philosophy and objectives as well as Health System initiatives.
EDUCATION/TRAINING:
Bachelor of Science in Nursing or Bachelor of Science in Dietetics or related field from an accredited facility.
LICENSURE/CERTIFICATION:
Current Registered Nurse licensure from the Illinois Department of Professional Regulation or Registered by the Commission of Dietetic Registration the credentialing agency for the Academy of Nutrition and Dietetic and currently licensed in the State of Illinois.
BLS certification is required.
Certification as a diabetes educator with experience in education preferred. If not currently certified will obtain Certified Diabetes Care and Education Specialist (CDCES) certification after accumulating the required 1000 contact hours to sit for the exam.
EXPERIENCE/SKILLS/ABILITIES:
Excellent interpersonal skills and ability to effectively deal with all levels within the organization and community. Strong verbal and written communication organizational and critical decision-making skills. Successful completion of annual clinical age specific and job specific competencies and skills validation tools.
