Job Summary:
This role is responsible for delivering high-quality person-centred social work practice across adult social care supporting individuals with complex needs to achieve independence safety and well-being.
The Social Worker will manage a varied caseload conduct assessments and implement care plans in accordance with relevant legislation such as the Care Act 2014 Mental Capacity Act 2005 and associated safeguarding protocols.
Working collaboratively with internal teams external partners and the wider community the post holder will support vulnerable adults and their carers in accessing the services and interventions needed to live fulfilling independent lives within their local communities.
Key Duties/Accountabilities (Sample):
Carry out strengths-based person-centred assessments in line with the Care Act 2014 and related legislation.
Manage a caseload of adults with a variety of complex needs including mental health conditions and safeguarding concerns.
Coordinate support plans undertake regular reviews and ensure eligibility criteria are applied appropriately.
Conduct carers assessments and provide relevant support and guidance.
Work proactively with individuals and their families to promote independence resilience and informed decision-making.
Raise and progress safeguarding concerns in line with statutory procedures and participate in strategy discussions or meetings where required.
Maintain accurate case notes using digital case management systems and uphold confidentiality in accordance with data protection requirements.
Liaise with internal and external stakeholders including health providers and partner agencies to ensure coordinated service delivery.
Contribute to service improvement through collaborative team working performance monitoring and constructive challenge of existing practices.
Assist in financial assessments and advise on personal budgets or contributions toward care provision.
Uphold and promote best practice in line with professional standards maintaining Social Work England registration and continuous professional development (CPD).
Respond to urgent needs and manage risk to ensure safety and immediate support for residents.
Skills/Experience:
Thorough knowledge of adult social care legislation including the Care Act 2014 Mental Capacity Act 2005 and safeguarding frameworks.
Proven experience in managing a caseload and carrying out assessments and support planning.
Ability to work collaboratively across professional boundaries and build strong working relationships with individuals carers families and service providers.
Strong communication negotiation and problem-solving skills with the ability to mediate and manage challenging conversations.
Excellent time management and organisational abilities to meet statutory deadlines and priorities.
Competent in using IT systems and electronic case recording tools (experience with Liquidlogic IAS desirable).
Emotional resilience active listening and a reflective practice approach to maintain service quality and personal well-being.
Understanding of the financial implications of care planning and commissioning services with value for money in mind.

Additional Information:
Qualified social worker with a degree in Social Work (or equivalent) and current registration with Social Work England (SWE).
This post is subject to an enhanced DBS check.
