Job Description
ProMedica Physicians Group (PPG)is seeking a dedicated and compassionateAdvanced Practice Providerto join our healthcare team atProMedica Flower Hospitalin Sylvania OH.
HIGHLIGHTS:
- Schedule: 5 nights on 10 nights off.
- Weekend/ Call Requirements: Rotating coverage once a month
- Current team consists of 7 providers
- Support staff consists of 8 MDs 28 APPs 1 lead APP and a Practice Director
- Deliver comprehensive medical care to patients 18 years of age including conducting physical examinations diagnosing and treating acute and chronic conditions and prescribing medication to patients 18 years of age
- Develop individualized treatment plans based on patient history and medical assessments while collaborating with a multidisciplinary team to ensure high-quality coordinated care.
- Play a key role in educating patients on preventive care healthy lifestyle choices and disease management contributing to the overall well-being of the community. Essential for 360 base care that Levis IM providers the community.

Requirements
- Certificate of Authority to practice.
- Current American Heart Association (AHA) CPR certification(s) based on function and patient demographic.
- Current Ohio state license as a Nurse Practitioner or Physician Assistant
- Master of Nursing (MSN) or Doctor of Nursing (DNP) degreeor Masters degreefor PA
