This position will work in collaboration with the interdisciplinary treatment team and will provide support to the Social Services Specialist and Activities Coordinator. The Discharge Planner will be responsible for collaborating with treatment team to coordinate a successful discharge plan for patients.
This position facilitates a warm handoff to the next level of care for patients as they leave the hospital. The position will address barriers to treatment in the transition. Additionally the position will coordinate with new and established providers supporting patients during transitions in care.
Duties/Responsibilities:
- Gather collateral information from family or other caregivers and serve as a liaison between caregivers and treatment team to identify the needs of the patient and/or family.
- Provide daily updates to the Interdisciplinary Treatment Team on the patients preparedness for discharge.
- Coordinate and facilitate caregiver meetings with other treatment team members.
- Documenting all discharge planning activities in the medical record.
- Coordinate care between the Hospital Outpatient Leadership Team and community partners to support success for patients.
- Ensure both the mental health needs and physical health needs of the patient are being addressed and assist in scheduling needed follow up appointments.
- Coordinate placement in group homes residential treatment facilities local shelters nursing homes etc. as needed.
- Identifying patient and family needs for service and referring to community agencies and resources to address the needs and/or problems.
- Making patient transportation arrangements at the time of discharge.

- Proficiency in basic computer skills (i.e. data entry word processing and related applications) and ability to research and access information online.
- Demonstrates the interpersonal and communication (both verbal and written) skills necessary to communicate in a tactful sincere and effective manner with patients from a variety of backgrounds healthcare professionals various Hospital staff and agencies outside the Hospital.
- Ability to maintain professional composure in a high-stress office and patient care environment with rapidly shifting priorities and competing demands.
- Ability to work cooperatively with multi-discipline teams in situations requiring a high degree of flexibility creativity initiative while maintaining a positive attitude.
Required Qualifications:
- Education: High School Diploma or GED. Bachelors Degree preferred.
- Certificate or Professional License: No certificates or professional license required.
- Experience: Prefer two (2) years in a Behavioral Health setting.
- Valid drivers license and active liability insurance.
- Satisfactory results from criminal and driving record background checks
- Must be a minimum 21 years of age.
