Location:Prescot Lancashire
Working Hours:37.5 hours per week

We are seeking a proactive and compassionateBand 6 Occupational Therapistto join our NHS clientsFrailty Response and Acute Care Teamin thecommunity settinginPrescot Lancashire. This is a rewarding opportunity to support older adults and individuals with complex needs helping to prevent hospital admissions and promote independence at home.
Key Responsibilities
- Deliver high-quality occupational therapy assessments and interventions in patients homes and community settings.
- Work as part of a multidisciplinary frailty response team to provide rapid support to individuals experiencing acute health deterioration.
- Develop and implement person-centred care plans aimed at maintaining function safety and independence.
- Support discharge planning and admission avoidance through timely and effective interventions.
- Liaise with families carers and other health and social care professionals to ensure coordinated care.
- Contribute to service development audits and quality improvement initiatives.
- Provide supervision and support to junior staff and students as required.
Qualifications & Experience
- Degree or diploma in Occupational Therapy
- HCPC registration as an Occupational Therapist
- Experience working in acute care frailty or community settings
- Strong clinical reasoning communication and organisational skills
- Ability to work autonomously and as part of a multidisciplinary team
- UK driving licence and access to a vehicle for community visits (desirable)
