Job Description

Job Title:Occupational Therapist
Band:6
Location:Prescot Lancashire
Working Hours:37.5 hours per week
Rate:Competitive details to be discussed upon application

We are seeking a proactive and compassionateBand 6 Occupational Therapistto join our NHS clientsFrailty Response and Acute Care Teamin thecommunity settinginPrescot Lancashire. This is a rewarding opportunity to support older adults and individuals with complex needs helping to prevent hospital admissions and promote independence at home.

Key Responsibilities

  • Deliver high-quality occupational therapy assessments and interventions in patients homes and community settings.
  • Work as part of a multidisciplinary frailty response team to provide rapid support to individuals experiencing acute health deterioration.
  • Develop and implement person-centred care plans aimed at maintaining function safety and independence.
  • Support discharge planning and admission avoidance through timely and effective interventions.
  • Liaise with families carers and other health and social care professionals to ensure coordinated care.
  • Contribute to service development audits and quality improvement initiatives.
  • Provide supervision and support to junior staff and students as required.

Qualifications & Experience

  • Degree or diploma in Occupational Therapy
  • HCPC registration as an Occupational Therapist
  • Experience working in acute care frailty or community settings
  • Strong clinical reasoning communication and organisational skills
  • Ability to work autonomously and as part of a multidisciplinary team
  • UK driving licence and access to a vehicle for community visits (desirable)
