Position Summary
Functioning as part of the coordinated service delivery team the Outreach Worker works proactively with new and existing clients offering a range of support and recovery services within as existing service plan and on an as-needed basis. The Outreach Worker is a relationship-builder and advocate and provides services in a non-judgmental and culturally appropriate manner helping clients to build new skills and achieve their goals.
This Program works within an Assertive Community Treatment Model and utilizes the Housing First Principles from a social justice orientation which includes harm reduction pro-choice social determinants of health human rights social development population health health equality and trauma informed client centered practice.
Qualifications
Education
Valid drivers license and access to a reliable vehicle
Skills Expertise and Experience
Minimum of 2 years experience working in community preferably with program target population
Experience providing outreach with hard to reach population
Critical Skills
Proven knowledge and ability in relationship-building critical thinking facilitation decision-making problem-solving and conflict resolution skills

Demonstrated ability to prioritize and focus work-related activities to effectively and efficiently achieve service goals and produce expected results/outcomes

Post-secondary education in a Social Services field
