Are you a skilled Physical Therapist looking to make a significant impact in the lives of patients in the comfort of their own homes We are seeking a dedicated professional to join our Home Health division committed to delivering exceptional care to individuals recovering from illness injury or surgery. This full-time role is vital in creating a nurturing environment that supports patient rehabilitation and enhances their quality of life.
As a Physical Therapist in home health you will:
- Conduct comprehensive physical assessments and review medical histories to evaluate patients conditions.
- Develop and implement individualized treatment plans tailored to each patients unique needs and goals.
- Administer therapeutic exercises and training programs designed to maximize functional independence.
- Educate patients and their families on injury prevention wellness and ongoing health management.
- Monitor document and communicate patients progress toward their rehabilitation goals.
- Collaborate with a multidisciplinary team to ensure a comprehensive approach to patient care.
- Maintain accurate and timely documentation in compliance with healthcare regulations.
- Masters degree in physical therapy.
- At least one year of experience as a physical therapist in a home health setting.
- Current state licensure to practice as a physical therapist.
- Compliance with accepted professional standards and practices.
- Valid CPR certification for clinical roles.
- Valid drivers license with the ability to travel to various locations as needed.
