Description

The Long-Term Services and Supports (LTSS) area of the Human Services and Public Health (HSPH) department is seeking two Senior Social Workers to join the Childrens Out of Home Placement team. This team supports the health safety and well-being of children with mental health needs intellectual and/or developmental disabilities or related conditions who are in or have been approved for out-of-home placement. In these positions you will work within a multi-disciplinary team that includes the children parents/guardians residential care providers mental health professionals educators medical professionals and other social workers.

Location and hours:
These positions are hybrid and will require travel throughout Hennepin County other areas of Minnesota out-of-state and remote as job duties require. Work hours are Monday through Friday 8 a.m. to 4:30 p.m. While these positions are designated as hybrid based on current requirements hires must reside in or relocate to Minnesota or Wisconsin. New Employee Orientation (NEO) for all new benefit-earning employees is in-person in downtown Minneapolis. Details about NEO will be provided upon hire.

In these positions you will:

  • Provide ongoing case management including minimum monthly visitation with each child in their residential setting (foster home group home or residential treatment center).
  • Coordinate services to meet the childrens physical educational cultural medical and mental health needs in the least restrictive environment that meets the childs needs.
  • Consult with other social workers therapists care providers parents/guardians Guardians Ad Litem medical and/or mental health professionals and others about the childs needs.
  • Provide information for court and administrative reviews and attend court hearings.
  • Complete documentation in a timely manner including time reporting court reports service and placement plans and service authorizations.
  • Comply with federal and state regulations for case management of children with intellectual and developmental disabilities in placement and follow best practice and professional standards.
  • Review childs records to assist in determining and maintaining eligibility for services.

  • Experience:
    • Working with children who have developmental and/or intellectual disabilities physical limitation medical conditions emotional and/or mental health needs.
    • Providing case management or completing MnCHOICES assessments including knowledge of MnCHOICES Revision Assessments and Support Plans.
    • With Medical Assistance services waiver services Rule 185 case management and out-of-home placement.
    • Using the following programs: Social Service Information System (SSIS) Electronic Case Files (ECF) Medicaid Management Information System (MMIS).
    • Using Microsoft Office (Microsoft Word PowerPoint Excel OneNote and Outlook).
  • Knowledge of:
    • Social work best practices and ethics.
    • County and community resources that assist children with mental health intellectual and development disabilities or related conditions.
  • Ability to:
    • Demonstrate excellent interpersonal communication organizational and computer skills
    • Understand issues related to equity and to demonstrate a commitment to promote diversity and inclusion.
