Position Summary
The Gattuso Centre for Social Medicine and Population Health seeks to address health inequities while working alongside community organizations UHN clinicians marginalized populations persons with lived experience patients and all levels of policymaking to co-design evaluate and scale innovative data-driven programs that lead to better health. Applying a social determinants of health approach the Social Medicine and Population Health Program aims to bridge health services and the broader social environment of UHNs most medically and socially complex patients with the highest needs through the design implementation research and evaluation of new care models and initiatives to address the health implications of inequity.
The Social Medicine Mobile Health initiative aims to improve health equity by bringing low-barrier primary care supports directly to vulnerable communities within downtown Toronto. As an integral member of the Mobile Health Team the Mobile Health Service Support Representative/Driver will work alongside the Mobile Health clinical team to drive the Mobile Health Clinic to various locations within downtown Toronto. This role will serve as the primary driver of the Mobile Health Clinic assist the clinical team with the delivery of patient care services and support the clinic administratively.
