Position Summary

The Gattuso Centre for Social Medicine and Population Health seeks to address health inequities while working alongside community organizations UHN clinicians marginalized populations persons with lived experience patients and all levels of policymaking to co-design evaluate and scale innovative data-driven programs that lead to better health. Applying a social determinants of health approach the Social Medicine and Population Health Program aims to bridge health services and the broader social environment of UHNs most medically and socially complex patients with the highest needs through the design implementation research and evaluation of new care models and initiatives to address the health implications of inequity.

The Social Medicine Mobile Health initiative aims to improve health equity by bringing low-barrier primary care supports directly to vulnerable communities within downtown Toronto. As an integral member of the Mobile Health Team the Mobile Health Service Support Representative/Driver will work alongside the Mobile Health clinical team to drive the Mobile Health Clinic to various locations within downtown Toronto. This role will serve as the primary driver of the Mobile Health Clinic assist the clinical team with the delivery of patient care services and support the clinic administratively.

Duties

• Function as the primary driver of the Mobile Health Clinic to designated clinic locations within downtown Toronto ensuring compliance with all Ontario traffic laws
• Perform visual vehicle circle check inspections before and after each route/scheduled clinic location
• Prepare and set up the Mobile Health Clinic for service and perform clinic breakdown at the end of clinic. This includes setting up the awning clinic signage opening clinic doors as required and snow removal from the mobile clinic
• Efficiently manage predetermined clinic schedules and routes
• Assist patients by providing mobility support when coming on and off the Mobile Health Clinic as needed
• Responsible for maintaining the Mobile Health Clinic mechanical maintenance schedule and filling up the gas tank when fuel is low
• Train to assist with the patient registration/intake process in Epic Health Information System and adhere to strict confidentiality when handling patient information in accordance with Ontarios Personal Health Information Protection Act (PHIPA)
• Conduct post-service patient satisfaction surveys
• Liaise effectively with the Mobile Health Care Team to ensure smooth clinic operations support patient flow and foster a safe and collaborative care environment
• Performing other duties consistent with the job classification as required

Qualifications :

• Completion of High School education or recognized equivalent
• Must have 3 or more years of experience driving a cargo van (e.g. Mercedes Benz 3500 Sprinter Van or a similar vehicle)
• A valid G license is required along with an acceptable driving record (no major traffic violation at fault accidents or license suspensions in the last 6 years)
• A 3-year uncertified drivers record containing driver and license details list of conviction information (if applicable) any applicable demerit points and suspensions
• Intermediate-level experience in Microsoft Office 
• Experience in the Epic Health Information System preferred
• Excellent time management and organizational skills; ability to work under pressure
• Ability to interact with mobile health clinic patients and healthcare team members in a responsible professional and ethical manner
• Willingness to participate in additional training to support clinical operations digital systems (e.g. Epic) and best practices for patient engagement
