Job Description

We are hiring a Full-time Registered Nurse (RN) to serve as Case Manager for seriously ill patients.
As a case manager you will provide skilled nursing services in a patients residence which includes assessing planning implementing and evaluating phases of nursing care. You will work alongside your clinical and operational team members to ensure the provision of high-quality hospice services. Candidates must possess excellent assessment skills critical thinking skills and strong organizational abilities. Candidates should be energetic have strong communication and interpersonal skills and have a demonstrated ability to work independently and exercise sound clinical judgment.
Applicants must have
- 1-2 years of acute care experience; hospice or home care experience is strongly preferred.
- Must have current NCBON licensing without restriction.
