Lead Support Worker
West Hampshire
Permanent Full Time (37.5 hpw)
Pay 13.21 to 13.63 per hour (25838 to 26645 per annum)and great benefits including Health Cash Plan

Hampshire West is a Mental Health Supported Accommodation service which also delivers a community outreach service within the same footprint. Within our accommodation Hubs in the New Forest Winchester and Eastleigh we work with adults who experience severe and enduring Mental Health challenges.
Supporting individuals to develop greater self-reliance and independence enabling better self-management of personal mental health physical health and wellbeing. Within the community we support vulnerable people with complex needs providing short-term interventions linking customers to other services and professionals.
Youll be supporting customers with support needs around hoarding poor living conditions or difficulty maintaining their well-being. Youll make a big difference every day helping isolated customers in the community achieve their hopes and aspirations. Amazing we know!
Typical day as a Lead Support Worker (known by us as Coordinator)
- Working with customers with complex needs in their own homes encouraging them to engage and build trust with us and other agencies.
- Facilitate access to work education or training
- Create personal support plans and risk assessments with customers organise regular reviews and coordinate support with other professionals.
- Support customers to safely manage any substance misuse issues thoughts/feelings about and suicide and/or self- harm
- Helping customers with daily living activities such as making meals shopping budgeting maintaining their tenancy and moving onto independent accommodation.

You bring
- Passion to support our customers to live their best life working collaboratively with an eye for detail.
- Experience of creating person-centred support plans and supporting colleagues in working to work to the plans.
- Experience of coordinating and assessing customer referrals and make it count from the first contact.
- The ability to work on your own initiative manage own diary remain calm under pressure and have a resilient approach
- To get from A to B youll need a vehicle insured for business purposes. The great news is that well pay your mileage!

Our team
Youll join Emma and the Hampshire Westteam theres quite a few of us but dont worry we are a friendly bunch and try to keep that team feeling by getting to know each other at our monthly team meetings always with snacks and a brew to hand.
Were super passionate about what we do! Were proud of working with customers who may have fallen through the gaps and supporting them to get back on their feet and link in with other services.
Job details
- Were open to agreeing a work pattern with you. Where were flexible with you we do need to meet the needs of our customers. That may mean adapting your start or finishing times on occasion.
- Our accommodation service is delivered Monday to Friday 9-7pm & 10-6pm weekends and bank holidays.
- Our outreach service is primarily Monday to Friday we may ask for you to flex your hours of work or days in order to meet service needs or engage with that individual at a time that best suits them.
- Although youll lone work at time we do get together regularly for training and team meetings.
- Able to use technology for creating and updating support plans complete online learning and to collaborate with colleagues.
- Youll need an Enhanced DBS check done and we pay for that.
