Job Description
Unified Care Hospice Texas is seeking a compassionate and experienced Physical Therapist (PT) to join our interdisciplinary team. In this rewarding per diem role youll provide comfort-focused therapy to patients at the end of lifepromoting mobility relieving discomfort and supporting independence and dignity through gentle goal-oriented interventions.
What Youll Do:
Conduct physical therapy evaluations to assess mobility safety and comfort.
Develop individualized care plans centered on pain relief positioning and fall prevention.
Educate families on safe mobility techniques and equipment use.
Collaborate with the care team to support patients evolving needs and goals.
Focus on enhancing quality of lifenot rehabilitation.
What Were Looking For:
Graduate of an accredited DPT or MPT program.
Active PT license in the state of Texas.
Minimum of 2 years experience ideally in hospice palliative care or home health.
Empathy clinical skill and a passion for making a difference.
Valid drivers license and reliable insured vehicle.
