JOB PURPOSE:

Endeavors is seeking a compassionate and experienced Case Manager to join our Warrior Support Program which supports K9s For Warriors by addressing the social and mental wellness needs of veterans awaiting placement with a service animal. This role is ideal for a dynamic mission-driven professional who thrives in a collaborative fast-paced environment and is passionate about veteran services.

As the Case Manager you will complete and review needs assessments conduct suicide risk screenings and interventions and document service deliveries. This position also plays a critical role in ensuring effective referrals engaging community partners and supporting veterans as they prepare for the intensive on-campus K9 training program. You will support efforts to meet program performance goals and continuously improve service delivery processes.

Qualifications

ESSENTIAL JOB RESPONSIBILITIES:

Conduct or review needs assessments and develop individualized care plans.
Conduct suicide risk screenings utilizing the Columbia Suicide Severity Rating Scale on a scheduled basis and support the creation of a safety plan when appropriate.
Refer Veterans to appropriate behavioral health services social resources and community-based supports.
Provide consistent follow-up to monitor service engagement and address barriers to care.
Document all services and case notes in accordance with program standards and data tracking requirements.
Participate in regular case conferencing training sessions and interdisciplinary collaboration.
Support data collection for performance metrics including tracking Veteran engagement outcomes.
Maintain strong working relationships with community providers VA contacts and collaborative partners.
Collaborates with K9s For Warriors clinical and intake team to understand case management needs and support any risk management and crisis intervention services as necessary.
Demonstrate exceptional customer service in everything you do by placing the child family Veteran or client first to support our mission to Empower people to build better lives for themselves their families and their communities.

ESSENTIAL QUALIFICATIONS:

EDUCATION: Bachelors Degree in Social Work Sociology Psychology or related field; Masters Degree preferred.

EXPERIENCE:

  • Minimum 1 years of case management experience.
  • Experience working with veterans is highly preferred.
  • Familiarity with suicide prevention practices and trauma-informed care is essential.
  • Experience collaborating with public health social services and veteran-serving organizations.
  • Strong interpersonal organizational and problem-solving skills.
  • Proficiency in case management and electronic recordkeeping systems.

LICENSES: Drivers License with clear record required.

VEHICLE: Must have daily use of a vehicle without prior notice.

OTHER: Must be available and willing to travel to various locations and with such frequency as the business need dictates. Must be available and willing to work nights weekends and holidays as required to meet business needs. Must not pose a direct threat or significant risk of substantial harm to the safety or health of himself/herself or others.
