Job Description

Make a Difference Every Visit

Unified Care Hospice Texas is seeking a compassionate skilled and mission-driven Occupational Therapist (OT) to join our interdisciplinary hospice team. This per diem role offers the opportunity to provide comfort-oriented care that preserves dignity and fosters independence for patients at the end of life.

Key Responsibilities
  • Conduct functional evaluations to assess patient needs in activities of daily living (ADLs) safety and comfort.

  • Develop and implement personalized comfort-focused care plans (e.g. ADL adaptation energy conservation positioning cognitive training).

  • Educate caregivers on safe patient handling adaptive equipment and techniques that promote comfort and function.

  • Collaborate with an interdisciplinary team including physicians nurses social workers and chaplains.

  • Adjust care strategies as conditions evolve prioritizing comfort over rehabilitation.

  • Accurately document evaluations and care in the EMR system.

  • Maintain compliance with HIPAA OSHA state and federal regulations.

Qualifications
  • Education: Graduate of an accredited Occupational Therapy program (OTD or MOT).

  • Experience: Minimum of 2 years preferably in hospice palliative care or home health.

  • Licensure: Active Occupational Therapist license in the state of Texas.

  • Other Requirements: Must be a licensed driver in Texas with a reliable insured vehicle.
