Whether youre looking for a flexible side opportunity or a meaningful way to reconnect with patient care this role offers autonomy purpose and the ability to make a real difference right here in Bergen County.

What Youll Do:

• Complete initial and ongoing assessments of clients health and home care needs

• Develop customized care plans in partnership with clients families and caregivers

• Supervise and delegate tasks to Certified Homemaker-Home Health Aides following NJ regulations

• Perform home visits to ensure care plans are followed (every 60 days)

• Educate clients and families to promote safety wellness and independence

• Maintain clear timely documentation.

• Collaborate with the full care team to ensure continuity and quality of care.

What Were Looking For:

• ActiveRN license in New Jersey(required)

• Minimum of2 years nursing experience home health preferred

• BSN preferred but not required

• Familiarity with NJ home care regulations

• Confident in supervising and educating CHHAs

• A kind patient-focused approach and strong attention to detail

• Reliable transportation and flexibility for Bergen County area visits
