With our service sites based in Lilydale we are passionate about tailoring unique programs and activities to empower people living with disabilities to live a fabulous life.

About the Role

As an Outcomes Lead at Community Connections you will play a pivotal role in mentoring and guiding a team of Disability Support Workers to deliver high-quality person-centered outcomes for the people we support. Your responsibilities include:

  • Understanding developing and refining individual programs of support in line with a persons NDIS goals
  • Developing links to a persons community that offer them greater opportunity for inclusion and the development of valued social roles
  • Onboarding supporting supervising and developing of assigned team members
  • Managing transport logistics during service delivery
  • Collaborating on service quality monitoring planning analysis and reporting
  • Working with the Leadership team to ensure smooth rostering and system updates
  • Collaborating with peers to maintain accurate person-supported information and documentation
  • Reviewing and approving Carelink entries in line with payroll and billing requirements
  • Manage and Follow Up on Incident Reports
  • Demonstrate incorporating practice leadership in daily operations
  • Delivering hands-on disability support work as required
  • Student placement supervision as required


Requirements

About You

You will:

  • Be a natural influencer modelling a positive attitude professional conduct and unwavering commitment to quality outcomes and service delivery for staff individuals with disability and the community
  • Possess a deep understanding of the NDIS its funding and systems
  • Have excellent administration organisation and time management skills
  • Have experience in leading and inspiring small teams
  • Have prior support worker experience including demonstrated understanding and knowledge of the tasks of a disability support worker
  • Be committed to service excellence human rights and zero tolerance to violence abuse neglect and exploitation
  • Have experience running a day service (desirable)

Requirements

  • Certificate IV in Disability desirable but not mandatory
  • A current NDIS Worker Screening and Working with Children Check (we can support you through the process of obtaining one or both checks if you dont already have them).
  • Current unrestricted drivers license
  • Current CPR and First Aid certification
  • High level professional written and verbal communication / presentation skills
  • High level organisational and time management skills including a proven ability to prioritise meet deadlines and work autonomously
  • Compliance with OHS&W regulations legislation and Policies and Procedures is required. Successful completion of pre-employment safety screening checks mandatory training and their subsequent renewals is necessary.
