About the role
As an Intensive Therapeutic Case Manager you will play a pivotal role in providing holistic support to young people in need. Your primary responsibility will be to work closely with young people who are experiencing a range of emotional behavioural and psychological difficulties.
To be successful in this role you will have experience working with Children & Young People in out of home care and an understanding of trauma and its effects on children and young people. This is a hybrid working role and regular travel across Grafton Lismore and Tweed Heads is a requirement of this position.
Key Responsibilities
- Develop and implement high quality therapeutic case plans for Children and Young People in LWBs care
- Develop and maintain functional and supportive relationships with children and young people in therapeutic care - Significant Disability as well as their families to support positive outcomes
- Attend shifts within the intensive therapeutic care homes and work closely with the team
- Meet with clients and family members to gather input for case plans and ensure their views are represented
- Coordinate and network with other agencies to effectively achieve case plan goals within tight time frames.
- Participate in an on-call roster as required
- Ensure compliance with all internal policy and procedure statutory and contractual requirements in relation to management of service delivery
Skills & Experience
- Relevant bachelors degree or a relevant diploma and be working towards a degree. (A qualification is desirable but not essential for Aboriginal and/or Torres Strait Islander applicants with relevant experience and a willingness to undertake further training)
- Previous case management experience within an Intensive Therapeutic Care or Out Of Home Care (OOHC) setting
- Experience supporting Children & Young People with disability who have experienced backgrounds of trauma abuse and/or neglect.
- Understanding of current casework practices for children and young people
- Current drivers licence
Successful candidates will be required to clear probity checks including National Criminal History Record Check NSW Working with Children Check & Residential Care Workers Check
