Developmental Pathways (DP) has an opportunity for a Douglas CountyLiaison on ourCommunity Engagement team! This position is responsible for driving high-impact service delivery initiatives managing cross-functional projects and serving as a trusted liaison between Developmental Pathways and assigned county systems ensuring streamlined communication and strategic collaboration across departments providers and regulatory agencies. The Liaison will provide advanced-level leadership in care coordination systems navigation and cross-sector collaboration to optimize outcomes for children and adults receiving services.

The ideal Douglas County Liaison will possess deep expertise in human services systems excel in navigating complexity and demonstrate an unwavering commitment to advocacy inclusion and quality of life for individuals served. This is an opportunity to complete presentations for various community partners in our service area to inform about DP services and supports.

This role offers a 4-day 40-hour workweek Monday-Thursday in a hybrid environment with the flexibility to work remotely as well as the opportunity to collaborate in-person at our headquarters office in Aurora CO and off-site locations. Occasional work on evenings and weekends will be needed.

  • A minimum of two (2) years experience in human services field such as service coordination or case management
  • Demonstrated success in cross-systems navigation advocacy or project implementation
  • Personal vehicle in good operating condition for use during work as appropriate
  • Valid Colorado motor vehicle license and proof of motor vehicle insurance
  • Ability to meet our driving requirements - no suspensions/ no more than two tickets in the past three years
  • Upon request qualifying need and approval driving requirements may be waived in lieu of reliable transportation.
    Preferred
    • One (1) year of public speaking and/or presentation experience
    • Able to speak write and interpret fluently in English and another language based on the business needs
    • Support with written translation of internal documents for use by stakeholders as appropriate

    Additional Responsibilities
    Strategic Thinking & Problem Solving:
    • Use critical analysis to anticipate service gaps and proactively implement innovative solutions for complex issues affecting individuals and families
    • Regularly support human services caseworkers and teams to navigate resources for youth who have been abused or neglected. Possess the ability to think past the harmful situation the child has faced to determine how to best meet the needs moving forward
    • Ensure strategic alignment of services policies and stakeholder communications within county jurisdictions while championing equity compliance and person-centered practices
    Relationship & Stakeholder Management:
    • Provide holistic care coordination and case consultation by assessing needs facilitating interdisciplinary team (IDT) meetings and aligning services with individual priorities eligibility criteria and funding guidelines
    • Act as a key point of contact for high-priority or complex cases including crisis navigation and multi-system coordination
    • Guide individuals and families through intake eligibility determination and service enrollment processes with clarity and compassion
    • Cultivate and sustain collaborative relationships with individuals families peers providers advocacy groups county agencies and state partners to build a responsive and coordinated network of support
    • Establish and sustain productive partnerships with law enforcement mental health
    Communication & Influence:
    • Communicate with clarity purpose and cultural responsiveness across a variety of audiences. Effectively negotiates and mediates to ensure equitable access to resources and services
    • Mentor and provide peer consultation to staff and human services agencies contributing to knowledge sharing onboarding and professional development
    • Deliver public presentations and community education sessions about the disability system services and access pathways to raise awareness build partnerships and empower stakeholders
    • Lead and/or contribute to multidisciplinary projects and initiatives involving intake eligibility crisis response and community-based support models
    Regulatory Expertise & Compliance:
    • Maintain comprehensive knowledge of federal state and local regulations. Ensure services are delivered in alignment with Department of Healthcare Policy and Financing (HCPF) requirements as well as DP policies and ethical standards
    • Montori and maintain accurate documentation and data systems to ensure timely reporting transparency and continuous quality improvement
