4 months contract with local authority
We are seeking a dedicated and experienced Social Worker/Social Care Assessor to join our team and support the discharge planning process for patients. The successful candidate will be responsible for conducting person-centered assessments planning for the provision of support and services and ensuring the safeguarding of vulnerable adults. This role requires a thorough understanding of NHS & Social Care Legislation eligibility criteria and the ability to work effectively within a multi-disciplinary team.
Responsibilities
- Conduct person-centered assessments and plan for the provision of support and services to assist patients in meeting their outcomes and living independently.
- Ensure accurate and timely recording of all information/assessment activity onto the Social Care records database.
- Gather sufficient information about patients to accurately assess their needs against eligibility criteria.
- Identify and manage risks for individuals in accordance with Council policy and support patients to manage risk appropriately.
- Participate in Specialist Response/Duty Rota and work effectively as part of a multi-disciplinary team.
- Adhere to Council corporate and departmental Policies and Procedures at all times.
- Contribute proactively to the effective working of the team and engage in continual professional development.
Requirements
- Proven experience as a Social Worker/Social Care Assessor
- Knowledge of NHS & Social Care Legislation and eligibility criteria
- Ability to conduct person-centered assessments and manage risks for individuals
- Strong communication and interpersonal skills
- Ability to work effectively within a multi-disciplinary team
- Registration with the Health and Care Professions Council
- Commitment to continual professional development and upholding professional standards
