The Behavioral Health Case Manager (BHCM) serves as a member of a multi-disciplinary patient care team for NATIVE HEALTHS Behavioral Health Department. The BHCM provides case management services that support the treatment goals of the patients care plan. The Behavioral Health Case Manager represents the company in a professional and positive manner to enhance and promote the core values and mission of NATIVE HEALTH always exercising the utmost discretion diplomacy and tact in customer interactions.
KEY ACCOUNTABILITIES:
- Provides non-intensive case management services in support of Behavioral Health patients and providers.
- Supports the behavioral health program and its team members.
- Engages in quality improvement and quality assurance activities for the department.
- Supports the organization as needed.

Qualifications :
- A bachelors degree in social work or a related behavioral health field is required. A masters degree in social work or a related field is preferred.
- Two years of relevant work experience.
- Bilingual in Spanish is highly preferred
- Experience working with underserved and diverse populations is preferred.
Standard Employment Requirements:
- Must possess a valid and unrestricted Arizona drivers license (within 30 days of hire if applicant is from out-of-state) with no DUI/DWI or reckless driving convictions in the last five years. No more than two at-fault accidents in the last three years. Must maintain a valid Arizona drivers license during employment. Must be insurable under NATIVE HEALTHs liability auto policy.
- Must pass a criminal background check and obtain a Class I Fingerprint Clearance Card within the initial ninety (90) calendar days of employment and retain a valid card while employed with NATIVE HEALTH.
- Must pass substance abuse testing upon employment and submit to random testing during employment.
- Must provide an updated immunization (IZ) record at the time of employment.
- Must have a current (within 12 months) Tuberculosis (TB) skin report upon employment and provide an annual update.
- Must obtain Basic Life Support (BLS) certification within (90) calendar days of employment and maintain certification during employment.
