Job Description
Join our dynamic Home Health team as a dedicated Physical Therapist where your expertise will be pivotal in transforming the lives of patients recovering from illness injury or surgery. This full-time role focuses on delivering exceptional patient-centered care directly in the comfort of their homes.
Key Responsibilities:
- Conduct comprehensive physical assessments and review medical histories to evaluate patients conditions.
- Create and implement personalized treatment plans tailored to each patients unique needs and objectives.
- Administer therapeutic exercises and training programs to enhance patient recovery.
- Provide education to patients and their families on injury prevention wellness and the recovery process.
- Monitor document and report on patients progress towards their rehabilitation goals.
- Collaborate with multidisciplinary healthcare teams to ensure comprehensive patient care.
- Maintain accurate and compliant patient records and documentation.
Qualifications:
- Masters degree in Physical Therapy.
- Minimum of 1 year of experience in home health as a physical therapist.
- Current physical therapy license within the state of practice adhering to professional standards and practices.
- Valid CPR certification maintained throughout the clinical tenure.
- Licensed driver with the ability to travel to various business locations.
