Your new role
We are currently seeking a flexible and enthusiastic Care Finder Case Manager to join our dynamic team covering Hobart and surrounding area on a Full Time basis. We are seeking candidates with excellent communication skills who can build rapport with those who are socially isolated within the community. Care Finder Case Managers provide assertive outreach to assist older people who face barriers to accessing aged care and are vulnerable marginalised and disadvantaged.
