Job Description

Summary

Position Summary
The Coordinated Care Management Enrollment Specialist is responsible for identifying contacting and enrolling eligible patients into the Chronic Care Management (CCM) program. This position plays a vital role in ensuring patients understand the benefits of CCM services and successfully initiate their participation.

Essential Duties

  1. Proactively contacts eligible patients to conduct informative persuasive and supportive enrollment calls.
  2. Accurately explains the benefits and structure of the CCM program customizing the message to meet individual patient needs.
  3. Ensures all enrollment documentation is completed thoroughly and uploaded to the patients electronic medical record.
  4. Maintains detailed call logs and tracks progress in managing assigned patient queues.
  5. Expedites enrollments efficiently meeting daily and weekly performance targets.
  6. Works designated patient lists independentlyensuring completion within required timelines.
  7. Collaborates with team members and leadership through regularly scheduled meetings and status updates.
  8. Utilizes multiple software platforms including Microsoft Office Suite (Outlook Excel Word Teams) CCIQ and electronic health records.
  9. Ensures all work is performed in compliance with HIPAA and organizational privacy/security policies.
  10. Actively engages patients via outbound and inbound calls.

Minimum Qualifications

  1. Education: High school diploma/GED and successful graduation from an accredited Medical Assistant program.
  2. Work Experience: 2 years of customer service or medical assistant experience required.
  3. Special Training Certification or Licensure: None

Salary Range: $25.04 - $33.11
