POSITION SUMMARY:

The FAC is the liaison between the Adult Day Health (ADH) program and the family and participant. The FAC works closely with the Adult Day Health Outreach Coordinator to provide tours complete enrollment documentation and complete all required assessments upon enrollment (annually). Coordinates the care planning process facilitates individual and group activities as requested and engages in ongoing communication with staff regarding the participants and their families. The FAC provides back-up coverage for the Site/Center Supervisor when necessary.


PRINCIPAL DUTIES AND RESPONSIBILITIES:

  • Works closely with the Adult Day Health Outreach Coordinator to provide tours; complete the enrollment application financial eligibility documents social service assessments enrollment and yearly testing and follows the inquiry and referral follow-up protocols as necessary.
  • Supports the Adult Day Health Outreach Coordinator in performing various outreach activities including staffing health events providing educational events to the community and meeting with referral sources. Responsible for supporting the achievement of Adult Day Health census goals as outlined in the Adult Day Health Outreach Plan.
  • Schedules care plans coordinates care planning process and assures timely completion of initial and yearly care plans; and coordinates and completes the discharge process including required documentation. Provides program staff with important participant and family updates to facilitate their ability to support and care for the participant.
  • Participates in site leadership team meetings facilitates caregiver support groups holds family meetings and leads individual and group activities (as requested). Maintains close communication with participants and their families.
  • Assists families in accessing needed services and supports through referrals to community resources as well as other LIFE programs including Care Management and SeniorLine.
  • Orients and supervises students volunteers and interns as requested.
  • Demonstrates a thorough understanding of HIPAA/HITECH laws and the related policies and procedures within the agency. Attends and successfully completes mandatory HIPAA/HITECH training at least annually.
  • Participates in on-going Commission on Accreditation of Rehabilitation Facilities (CARF) accreditation procedures of LIFE LSS & AFFILIATES.
  • Attends and participates in staff meetings in-services and projects and committees as assigned.
  • Completes required training as assigned in Relias.
  • Adheres to and support the facilities policies practices and procedures.
  • Accepts assigned duties in a cooperative manner and perform all other related duties as assigned by Supervisor.
  • Participates in special projects and performs additional duties as required or assigned.
Requirements

SKILLS: Excellent verbal and written communication skills. Experience working with individuals with memory and cognitive impairments. Positive professional attitude and appearance. Proficient in Microsoft Office programs including Word and Excel; ability to learn and use market specific software programs. Strong organizational skills with the ability to multi-task.


EDUCATION AND EXPERIENCE: Minimum of two (2) years of experience in geriatrics in a social service capacity.


PHYSICAL REQUIREMENTS: Requires constant hand grasp and finger dexterity; frequent sitting standing walking and repetitive leg and arm movements occasional bending reaching forward and overhead squatting and kneeling. Ability to communicate verbally with an excellent comprehension of the English language. Able to lift 30 lbs. for office work and marketing events; able to push up to 200 lbs. in a wheelchair. At least thirty percent of the work is performed while standing or walking. Good hearing and visual acuity.


CERTIFICATION LICENSES REGISTRATIONS: Clinical license or certification such as a CNA or LTC. Health care provider level of AED/CPR/First Aid. Tulsa County Food Handlers certification.


WORK ENVIRONMENT: The work environment is a day program health care setting. The activity level is high and the noise level moderate. Reasonable accommodations may be made to enable individuals with disabilities to perform the essentials functions.
