Job Description

The Halton Diabetes Program provides interdisciplinary care to individuals living with diabetes throughout the Halton area.  In particular the program offers assessment self-management support and care management to type 1 type 2 diabetes and pregnancy dialysis oncology emergency department acute inpatients and supports emergency and hospital discharges.  This role of Primary Health Care Nurse Practitioner will assist the program in responding in a timely manner to the higher acuity and complex patient population that has increased with the relocation of the program to the hospital. 

Reporting to the Manger Halton Diabetes Program and Diabetes Food Care Program the Nurse Practitioner will work within an interdisciplinary team to provide comprehensive health care for adult clients with complex diabetes conditions.  The Nurse Practitioner will practice at a high level of autonomy utilizing and demonstrating advanced knowledge and decision-making skills to conduct comprehensive assessments diagnoses in the management of the adult diabetes patients.  The incumbent will be the key liaison with primary care referral partners and the community to ensure seamless transitions facilitating collaboration among all partners involved in the ongoing care of the patient.  The role will be involved in quality improvement and administrative responsibilities participating in policy and procedure development as well as program development and evaluation activities. 


Qualifications :

  • Current registration in the Extended Class with the College of Nurses of Ontario required
  • Completion of a Masters Degree in Nursing or equivalent
  • Primary health Care Nurse Practitioner certificate completed
  • Member of the Nurse Practitioners Association of Ontario and Diabetes Canada
  • Demonstrated knowledge and experience (1-3 years) in the care of individuals with complex diabetes needs and associated comorbid conditions is required or equivalent
  • At least on year experience working in an advanced practice role
  • Certified Diabetes Educator designation required
  • Demonstrated ability and commitment to work as part of an interdisciplinary team and develop collaborative practices with medical personnel allied health staff and learners
  • Demonstrated experience working in chronic disease prevention and management including self-management approaches to care adult learning principles health coaching and case management skills
  • Exceptional interpersonal verbal and written communication skills and judgement when relating to patients families co-workers and members of the interdisciplinary team
  • Demonstrated ability to build and maintain effective relationships with a diverse range of people
  • Integration of the most recent research evidence into practice
  • Motivated self-learner who takes initiative and seeks learning opportunities to enhance patient care professional competence and support the team in evidence-based practice through consultation and education
  • Computer literacy in Meditech and other computer programs such as Microsoft Office
  • Effective organizational skills to support multi-tasking and setting priorities
  • Demonstrated excellence attendance record and able to meet the physical demands of the position


Additional Information :

Hours: Various rotating 7.5 hour shifts00 1200-2000 (hours are subject to change based on operational requirements)

Salary: $58.80 per hour - $72.01 per hour
