You will be updated with latest job alerts via emailThe Halton Diabetes Program provides interdisciplinary care to individuals living with diabetes throughout the Halton area. In particular the program offers assessment self-management support and care management to type 1 type 2 diabetes and pregnancy dialysis oncology emergency department acute inpatients and supports emergency and hospital discharges. This role of Primary Health Care Nurse Practitioner will assist the program in responding in a timely manner to the higher acuity and complex patient population that has increased with the relocation of the program to the hospital.
Reporting to the Manger Halton Diabetes Program and Diabetes Food Care Program the Nurse Practitioner will work within an interdisciplinary team to provide comprehensive health care for adult clients with complex diabetes conditions. The Nurse Practitioner will practice at a high level of autonomy utilizing and demonstrating advanced knowledge and decision-making skills to conduct comprehensive assessments diagnoses in the management of the adult diabetes patients. The incumbent will be the key liaison with primary care referral partners and the community to ensure seamless transitions facilitating collaboration among all partners involved in the ongoing care of the patient. The role will be involved in quality improvement and administrative responsibilities participating in policy and procedure development as well as program development and evaluation activities.

Hours: Various rotating 7.5 hour shifts00 1200-2000 (hours are subject to change based on operational requirements)
Salary: $58.80 per hour - $72.01 per hour
