About the Department:
The Weekend Well Check Support Staff focuses on conducting safety checks for participants who exhibit behavioral challenges. Promoting a secure and supportive atmosphere during weekend hours helps maintain a safe and stable environment.
Job Description:
The Weekend Well Check Support provides safety checks for clients with behavior issues under the supervision of the Adult Housing Program Supervisor and Director.
- Support clients in permanent supportive housing to promote residential stability
- Effectively advise public social services programs and assist clients with the applications and follow-up to ensure services were received
- Assist clients with shopping laundry and moving into an apartment which may include bending squatting lifting pushing pulling and simple grasping
- Provide follow-up services that intensive case managers may request
- Connect clients with wrap-around services through other programs offered at HELP including Workforce Services Job Readiness Workshops Energy Assistance Program holiday programs etc.
- Connect clients with mainstream programs including but not limited to Medicare and SSI/SSDI.
- Provide clients with referrals and resources for other social services and partner agencies.
- Communicate effectively and maintain boundaries with individuals from a variety of social cultural and economic backgrounds.
- Enter case notes in into HMIS daily reflecting activities of the clients progress and services provided
- Provide transportation for lower functioning clients ensuring appointments are kept and those clients are receiving all services required to achieve their goals on their Case Plan
- All other related duties as assigned by management for the needs of HELP of Southern Nevada
The ideal candidate will possess the following knowledge skills and abilities:
- Knowledge of local social service agencies programs and issues as they pertain to the local unhoused population
- Ability to remain ethical in all aspects of business dealings
- Knowledge of applicable statutes ordinances and codes.
- Knowledge of social service work and interviewing/referral techniques
- Ability to analyze situations accurately adopt an effective course of action exercise and use appropriate judgment in crisis situations
- Communicate effectively with persons from a variety of social cultural and economic backgrounds and possess a demonstrated understanding of and sensitivity to culturally diverse populations.
- Effective written verbal and listening skills. Independently initiates and responds to correspondence.
- Ability to be flexible quickly adjust to shifting priorities while remaining organized
- Establish and maintain cooperative working relationships with clients fellow employees other agencies and the general public
- General computer knowledge and the ability to use Microsoft Office software
- Ability to work flexible hours as required by the client caseload
Required Qualifications:
- Proof of Highschool Graduation or GED
- Active Nevada drivers License with clean MVR
- 2 years experience with data entry compliance or related field
- Previous experience working with unhoused population
Preferred Qualifications:
- Bachelors Degree in Social Work or related field
