Job Description
Job Title: Adult Social Worker - Hospital Discharge
Location: Barnsley (Hybrid)
Pay Rate: 35 per hour
Liquid Personnelis currently recruiting for anAdult Hospital Social Workeron behalf of our client. This is an exciting opportunity to join a dedicated hospital discharge team where you will play a vital role in supporting adults during their transition from hospital to home or other care settings.
Responsibilities :
- Experience working with health partners or hospital social work discharge teams.
- Strong social work values that guide professional practice.
- Focused on helping individuals achieve the best outcomes following hospital discharge.
- Committed to promoting independence through a strengths-based approach.
- Collaborates effectively with individuals families carers and communities.
- Supports informed decision-making processes.
- Works closely with health colleagues under the hospitalsDischarge to Assessprocess.

Qualifications & Experience:
To be successful in this role you must have
- Social Work England registration
- Eligible to work in the UK
- Hold a full UK driving licence
- Post-qualified experience
- Degree level or equivalent in Social Work
