Job Description
Join our amazing team of healthcare providers. We have been in business for over 10 years. As we expand we are looking for highly motivated LVNs to join our Private Duty Nursing team.
Essential Duties and Responsibilities
- Deliver direct patient care as assigned using established procedures in the patients home environment.
- Traveling to patients homes and managing their care plans according to physicians instructions
- Maintain accurate detailed reports and records on medical information vital signs or changes in patients conditions.
- Monitor patients medication usage and results.
- Document patients medical and psychological histories physical assessment results diagnoses treatment plans prescriptions or outcomes
- Evaluate patients behavior to formulate diagnoses or assess treatments.
- Consult and coordinate with healthcare team members to assess plan implement or evaluate patient care plans.
- Provide personal assistance medical attention emotional support or other personal care to patients.
- Train other Registered Nurses coordinate with other home health professionals caring for the patient and communicate the patients needs to the family providing support.
- Other duties as assigned.
