Clinical Support Specialist

Location: This role enables associates to work virtually full-time with the exception of required in-person training sessions providing maximum flexibility and autonomy. This approach promotes productivity supports work-life integration and ensures essential face-to-face onboarding and skill development.

The Clinical Support Specialist is responsible for providing day-to-day support to care team (including RNs NPs Pharmacists and Social Workers) and providing care coordination for patients.

How You Will Make an Impact

Primary duties may include but are not limited to:

  • Arranges and coordinates prescribed laboratory tests and/or diagnostic procedures for patients.
  • Procures supplies and equipment in a timely manner to support patient independence in the home.
  • Supports order delivery and receipt of Durable Medical Equipment (DME) and medical supplies as identified by patient care team.
  • Facilitates care coordination with external medical providers and specialists.
  • Identifies and schedules referrals with external providers including specialty and home health care.
  • Assists patients with scheduling visits with interdisciplinary care team members as necessary.
  • Assists with and provides non-clinical activities related to HEDIS expectations as determined by quality management leadership.
  • Manages inbound and outbound inquiries and document all correspondences within relevant systems timely.

Minimum Requirements:

Requires a H.S. diploma or equivalent and a minimum of 1 year experience or any combination of education and experience which would provide an equivalent background.

Preferred Skills Capabilities and Experiences:

  • Understanding of Medicaid/Medicare coverages of DME and supplies strongly preferred.
  • Certified Nursing Assistant Medical Assistant or Licensed Practical Nurse preferred.
