Job Description

Function/Duties of Position

  • The appointee shall provide clinical social work services as assigned by the supervisor in furtherance of The Hemostasis & Thrombosis Centers missions of advancing wellness and empowering possibilities for those affected by bleeding and clotting disorders through excellence in patient care teaching research and advocacy.
  • Provide social work services to all patients and families served by The Hemostasis & Thrombosis Center (HTC) at Oregon Health & Science University through both in-person and telehealth visits.
  • Conduct psychosocial assessments of patients and their families as a member of an interdisciplinary team. Based on assessment appointee participates in treatment planning intervention and evaluation to improve health outcomes and health-related quality of life.
  • Evaluate social determinants of health for patients served by the HTC and intervene appropriately to reduce barriers to care and increase health care equity.
  • Participate in treatment team meetings with various health care staff to provide consultation and/or psychosocial assessments that are used in establishing treatment goals identify resources and make appropriate referrals.
  • As appropriate provide social work services in patients homes schools bleeding disorder camp and other community-based settings.
  • Refer patients/family to community resources for assistance as needed and facilitate access to resources as well as maximal use of those resources.
  • Advocate for patients and families within the medical setting and community.
  • Provide crisis intervention short- and long-term counseling to patients and/or their families. Provide supportive counseling to reinforce patients strengths and assist them in dealing with the stress and feelings of grief fear and anger associated with acute and chronic illness injury hospitalization or disability.
  • Provide education clinical supervision and support of Social Work students including all facets and stages of clinical education for Social Work students and clinical education for trainees from related discipline. Provide education supervision and support of NICH (Novel Interventions in Childrens Healthcare) interventionists.
  • Participate in HTC Mountain States Regional Network national bleeding disorder community education and conferences.

Required Qualifications

  • A Masters Social Work (MSW)
  • Certificate of Clinical Social Work Associate (CSWA) or Licensed Clinical Social Worker (LCSW) required for employees who practice clinical social work.
  • Clinical experience working with individuals with chronic health care needs.
  • Experience performing psychosocial assessment of individuals with bleeding disorders.
  • Experience working on a clinical inter-disciplinary team.
  • Experience working with NICH Interventionists.
  • Ability to work independently and prioritize
  • Ability to supervise students and NICH Interventionists
  • Strong Verbal and written communication skills
  • Knowledge of community-based resources for families
  • Organizational skills to assure timely follow-up with families
  • Understanding of systems of care for individuals with bleeding disorders.
  • Positive collaboration and team-building skills
  • Understanding of diverse cultural geographic economic and social issues that impact families and ability to work effectively with diverse families

Preferred Qualifications

  • Health care experience preferred.
  • Clinical experience with bleeding and clotting disorders.
