The Occupational Therapist II-Home Care is responsible for patient care delivery in a home care environment and will provide the following services: assessment and evaluation of factors that influence occupational performance; formulating conclusions regarding patient/client needs and priorities to develop a patient/client centered intervention plan; and selecting and implementing evidence based interventions to support participation in areas of occupation. Responsible for overseeing the assignment of patients record keeping and the overall coordination of the units daily activities and provides home treatment education for patients families students co-workers and self-professional advancement. Performs other professional duties associated with the care of patient development of therapy program services and engagement and development of interdisciplinary teams.

Education Qualifications
- Graduate (e.g. bachelors masters or doctorate) from an accredited Occupational Therapy program which meets the licensure requirements in the state of practice is required.
Experience Qualifications
- 2 years of Occupational Therapy experience is required.
Certifications Licenses and Registrations
- Illinois: Licensure or Eligibility for licensure as a registered occupational therapist by the State of Illinois under the condition that they have been issued a letter of authorization from the Illinois Department of Financial & Professional Regulation (IDFPR) allowing them to practice under supervision is required.
- Wisconsin: Licensure or Eligibility for licensure as a registered occupational therapist by the Occupational Therapists Affiliated Credentialing Board in Wisconsin under the condition that they have been issued a temporary license allowing them to practice under supervision is required.
- Certified Basic Life Support (BLS) or completion in the first 90 days of employment is required.
- Valid state drivers license current automobile insurance and reliable automobile is required.
- Pass the National Board for Certification in Occupational Therapy (NBCOT) exam is preferred.

Pay Range:
$35.92 - $53.88
A successful candidates actual pay rate will be based on several factors including relevant experience skills training certifications and education.
