Job Description:
We are seeking a Full Time RN Case Manager to join our team in Vancouver WA. The ideal candidate will have experience as a full time case manager in hospice care. The RN Case Manager will be responsible for coordinating and managing the care of patients in a hospice setting ensuring that their needs are met and that they receive the highest quality of care.
Skills and Qualifications:
- Active RN license in the state of Washington
- Minimum of 2 years experience as a case manager in hospice care
- Strong communication and interpersonal skills
- Ability to work independently and as part of a team
