You will be updated with latest job alerts via emailThe Case Manager leads care coordination for young adults ages 18 to 24 transitioning from inpatient behavioral health treatment into a short-term 90-day housing program. Serving as the primary point of contact this role conducts needs assessments develops individualized service plans and collaborates with each participant to identify and engage their broader care team. The Case Manager ensures consistent wraparound coordination across housing and behavioral health systems facilitating access to services that promote stability and recovery. This position reports to the Manager of Youth Haven and Bridge Pointe Housing Services and ensures service delivery meets program objectives and agency standards.
essential duties
Program Delivery
Manage a caseload of up to 10 youth serving as their main service coordinator.
Conduct thorough needs assessments and create tailored service plans with each participant.
Organize and lead care team meetings to align efforts among all service providers.
Maintain regular contact with youth to track progress and revise plans as needed.
Participate in weekly staff meetings to share updates discharge plans and recommendations.
Provide case updates in weekly supervision with the Program Manager to ensure alignment with program goals.
Assist with daily operations including floor coverage and youth supervision.
Participate in the on-call rotation to manage referrals intakes and urgent coordination.
Case Management Tasks
Complete assessments service plans updates and discharge summaries per program and contract requirements.
Identify service gaps and make timely referrals to housing behavioral health and stabilization resources.
Communicate consistently with external providers to ensure continuity of care.
Facilitate connections to community resources and aftercare support.
Document all case activities promptly; maintain accurate and confidential records.
Support internal team growth through onboarding consultation and training as needed.
Represent the program in external provider meetings and support outreach initiatives.
Job Requirements
a. Education and Experience
Bachelors degree in a human services field is required.
At least 2 years of experience working with youth young adults or families facing homelessness.
Background in trauma-informed case management and youth development.
Experience handling conflict and crises within family systems or interdisciplinary teams.
b. Additional Requirements
Must pass a criminal background check.
Must pass a tuberculosis (TB) screening.
Must have reliable transportation and a valid Washington State drivers license (or obtain one by start date).
Must meet safe driving criteria as set by Friends of Youth.
Must maintain personal auto liability insurance for agency-related travel.
Must complete certifications in CPR First Aid and Bloodborne Pathogens within 30 days of employment.
Proof of COVID-19 vaccination is mandatory; no exemptions accepted.
